Initialization phase:

Hair follicles are tiny organs connected to fine loops that provide blood flow, nervous connection, and lymphatic drainage. Initialization happens nearly instantly. 

Rejuvenation phase:

In this phase of activation, we must diligently apply the sessions every week. Only after 4 to 8 sessions, depending on the program, can we space them out if long-term protection is needed.

The primary reason hair treatments fail is the inability to identify the correct cause and type of hair loss. MATU AUGŠANAS ZINĀTNE.

Matu izkrišana un plikpaurība ir plaši izplatīta problēma. Vairāk nekā 66% vīriešu piedzīvos matu izkrišanu līdz 35 gadu vecumam. Līdz 50 gadu vecumam 85% vīriešu un 50% sieviešu kļūst ievērojami plānāki mati. Bet, pirms aplūkojam risinājumu, iedziļināsimies matu augšanas procesā.


Mati aug mazos dzīvos maisiņos, kurus mēs saucam par folikuliem. Matu folikuls ir caurulei līdzīga struktūra (pora), kas ieskauj mata sakni un šķipsnu. Folikula pamatne izliekas, veidojot matu spuldzi, un to iepludina saistaudi, kas pazīstami kā ādas papilla. Ādas papilla satur daudz sīku asinsvadu un nervu izvirzījumus, kas baro folikulus (1. attēls). Šie folikuli veido matus, nedzīvu materiālu, kas izgatavots no keratīna. Tas pats materiāls, kas mūsu nagiem. Mēs esam dzimuši ar vairāk nekā vienu miljonu matu folikulu uz mūsu galvas, no kuriem aptuveni 100 000 pieaugušā vecumā izaugs, lai segtu mūsu galvas ādu. Vairāki apstākļi var ietekmēt matu folikulu kvalitāti.

Matu augšana folikulā ir cikliska, un to var iedalīt četrās fāzēs (2. attēls). Pirmā fāze ir augšanas fāze jeb anagēns, un tā ir visilgākā un ilgst gadus.

Otrā fāze ir pārejas fāze jeb katagens, un tā ir īsākā un ilgst vairākas nedēļas. Trešajā fāzē jeb telogēnā mati sāk izkrist 3 līdz 6 mēnešu periodā – izkrišanas fāzē. Ceturtā fāze ir eksogēns (faktiski agrīna nākamā anagēna fāze), kurā jauni mati izstumj vecos, un jauni mati gatavojas ieņemt to vietu.


Jo garāka būs anagēna fāze, jo biezāki un garāki būs mūsu mati. Jauniem un veseliem cilvēkiem ir gara anagēna fāze, kas lēnām pāriet uz telogēna fāzi. Vairāki apstākļi var ietekmēt katras cikla fāzes ilgumu.

Biezi un gari mati ir rezultāts gan veselīgam matu folikulam, gan labi funkcionējošam matu ciklam, jo īpaši ilgam anagēna fāzei.

Apstākļi, kas ietekmē matu ciklu

Novecošanās, stabila hormonālā līmeņa pazemināšanās apvienojumā ar ģenētisko predispozīciju, pakāpeniski pasliktinās matu folikulu dzīvības līniju. Tā rezultātā samazinās matu folikulu miniaturizācija, īsāka anagēna fāze un galu galā mazāk un plānāki mati. Matu folikulus, kas pāriet uz šo stāvokli, sauc par "guļošiem matu folikuliem". Šī iemesla dēļ guļošo matu folikulu palielināšanās ir visizplatītākais matu izkrišanas veids, ko sauc par AGA vai androģenētiskā alopēcija. Vīriešus un sievietes ietekmē šāda veida plikpaurība, lai gan katram ir savs modelis, kā parādīts AlviArmani diagrammā (3. attēls).

Ir normāli, ka katru dienu no mūsu galvas ādas izkrīt aptuveni 30–150 matiņu matu cikla ietvaros. Tas var atšķirties atkarībā no mazgāšanas un ķemmes tīrīšanas rutīnas. Mati ataug automātiski, tāpēc kopējais matu skaits uz mūsu galvas paliek nemainīgs.


Pēkšņa traumatiska pieredze var izraisīt ievērojamu matu skaita izkrišanu katru dienu. Liela matu daļa pāriet no augšanas fāzes (anagēns) uz izkrišanas fāzi (telogēnu). Parasti tikai 10% no galvas matiem ir telogēna fāzē, taču šajā situācijā anagēna fāze beidzas pēkšņi un mati nekavējoties pāriet telogēnā fāzē, kas palielinās līdz 30% vai vairāk. Tas notiek dabiski pēc bērna piedzimšanas vai pēc fiziska vai garīga stresa situācijām. Dažas slimības var izraisīt arī šo maiņu. Mēs to saucam par "TELOGĒNA EFFLUVIUM". The longer the anagen phase, the thicker and longer our hair will be. Young and healthy people have a long anagen phase that shifts slowly toward the telogen phase. Several conditions may affect the length of each phase in the cycle.

Thick and long hair is the result of a healthy hair follicle and a well-functioning hair cycle, especially a long anagen phase.

Conditions Affecting the Hair Cycle

Telogen Effluvium – Acute and Chronic

It is normal to shed approximately 30-150 hairs from our scalp daily as part of our hair cycle, which varies depending on washing and brushing routines. Hair regrows automatically, so the total number of hairs on our heads remains constant.

A sudden traumatic experience can result in a marked increase in the number of hairs shed each day. An increased proportion of hairs shift from the growing phase (anagen) to the shedding phase (telogen). Normally only 10% of the scalp hair is in the telogen phase, but in this situation, the anagen phase ends abruptly and hair shifts immediately into the telogen phase, which increases to 30% or more. This happens naturally after the delivery of a baby, or after physical or mental stress

situations. Some diseases can cause that shift as well. We call it the TELOGEN EFFLUVIUM. A very common type of hair loss. 

Depending on the causes, Telogen Effluvium can be ACUTE and limited to less than 6 months (as in the delivery of a baby)  or CHRONIC (as in longstanding stress and nutritional deficiencies). 

Anagen effluvium (AE)

Anagen effluvium refers to hair shedding that arises during the anagen or growth stage of the hair cycle. 

This is in contrast to telogen effluvium, or hair shedding, which occurs during the telogen, or resting, stage of the hair cycle.

Anagen effluvium is due to an acute injury to the hair follicles by an endogenous or exogenous cause, resulting in sudden diffuse shedding of structurally damaged hairs. Diffuse alopecia  (hair loss) may occur over days. The alopecia is non-scarring.

What causes anagen effluvium?

Any insult that impairs hair follicle keratinocytes' division and growth can cause anagen effluvium. Disruption to cell division in the hair matrix makes the hair narrow at its base and susceptible to breakage just above the zone of keratinisation.

The necrotic matrix forms plugs consisting of melanin, keratin and inner root sheath which are extruded through the follicular opening. This process is known as trichomalacia.

The main causes of anagen effluvium are an infection, a drug, a toxin, chemotherapy, radiation therapy, or an autoimmune disease.

 

Treatment of anagen effluvium involves addressing the underlying cause, focusing on hair care and possible medical interventions to support hair regrowth.

Conditions affecting the Hair Follicle

Aging, steady hormonal decline, combined with a genetic predisposition will gradually deteriorate the hair follicle's lifeline, resulting in miniaturization of the hair follicle, a shorter anagen phase, and ultimately less and thinner hair. Hair follicles shifting to this condition are called "sleeping hair follicles". An increase in sleeping hair follicles due to this cause is the most common type of hair loss, named AGA or ANDROGENIC ALOPECIA. Men and women are affected by this type of baldness, each with its pattern, as shown in the chart of Alvi Armani (Pic 4 of chart).

Degeneration of the hair follicle in AGA is gradual. It leaves us with a pattern of living, dead, and dormant hair follicles.

  • Hair follicles can be alive and active. In that case, you will have a luxuriant head of hair and will not need to do much. These living and active hairs are on average 0.1 mm thick.

  • Hair follicles can also "die" and no longer be able to produce hair. They unfortunately cannot be revived. A hair transplant can be useful in specific cases.

  • "Dormant" hair follicles are present in incipient baldness. They produce thin hairs (on average less than 0.04 mm thick) and appear as baldness from a distance. Baldness increases as more and more active hair follicles enter a dormant phase. Fortunately, with treatments, they can be transformed back into active hair follicles.

 

Hair loss through follicular regression is due to the degeneration of the vessels and nerves that run from the brain and heart to the hair follicle, and back. We call them the biofeedback loops. Lymphatics play a role, too. Nerves, blood vessels, and lymphatics deteriorate with aging, stress, poor lifestyle, diseases, and other external aggressions, as revealed during your first consultation. When the hair follicle fails to receive enough nourishment to its cells, the hair starts thinning and the follicle becomes dormant. Ultimately the hair follicle will vanish.

Balding men have on average 30.000 sleeping hair follicles. They become bald as hair thins out. This can go as slowly as a 1% loss a year up to a 30% loss a year. In these cases, baldness will appear soon in life, and immediate action is required to prevent baldness. Good technologies can result in a regression of AGA within 3 months of treatment if you have enough "dormant" hair follicles. This is the case in individuals aged between 25 years to 50 years with mild to moderate baldness corresponding to the framed heads on the Alvi Armani scale: Class 2 to Class 5.

Conditions Affecting the Hair Lustre and Vitality

Hair Root or Hair Shaft Damage 

The vast majority of people suffer from unmanageable hair that has lost its vitality and luster. Dull and unmanageable hair is often the result of overstyling, physical damage to the hair, perms, bleaching, or coloration. Extensions or wigs attached to the existing hair can also damage the roots. Compulsive pulling on the hair also falls into that category. 


 

Diseases of the scalp, or reflected on the scalp

Many diseases can impact hair loss or even directly affect the scalp tissue. Autoimmune diseases are at the core of alopecia areata in all its forms. Thyroid gland dysfunction and parathyroid gland overactivity can lead to hair loss. Inflammatory conditions, such as seborrheic dermatitis, eczema, or psoriasis, can also appear on the scalp, resulting in hair loss. Other inflammatory diseases can cause scarring, often leading to irreversible baldness. 


 

Others

Internal causes are equally important and include nutritional deficiencies, intoxications, some medications, and diseases that affect hair quality. As the causes may vary, so does the treatment.

From here on, we evolved to classification into 6 types:

  3. Telogen Effluvium Chronic (TEC) 

  4. Anagen effluvium (AE)
